Tadalafil is unusual among common erectile-dysfunction medicines because the same active ingredient can be used in two noticeably different ways. Some men take it when needed before sexual activity, while others use a lower strength every day; tadalafil is also used for urinary symptoms associated with benign prostatic hyperplasia, which means the reason for treatment can be just as important as the number of milligrams on the box.
It belongs to the PDE5 inhibitor group and increases the effect of nitric-oxide signaling in certain tissues. For erectile dysfunction this improves the blood-flow response to sexual stimulation, while the lower urinary tract effects are part of the reason tadalafil can also be prescribed for BPH.

What Tadalafil Treats
Tadalafil has more than one established use, and the dosing pattern changes according to the purpose of treatment. A low-dose daily product therefore should not be treated as the same purchase as a higher-strength tablet intended for intermittent use.
- Erectile dysfunction. Tadalafil can improve the ability to achieve and maintain an erection in response to sexual stimulation.
- Benign prostatic hyperplasia. Daily tadalafil can improve certain lower urinary tract symptoms associated with BPH.
- ED together with BPH. A daily regimen may be selected when both conditions are present.
- Pulmonary arterial hypertension. Tadalafil is also used in a different dosing context for pulmonary arterial hypertension, where treatment belongs under specialist supervision and should not be confused with ordinary ED products.
Daily Tadalafil and As-Needed Tadalafil
| Product Type | Dosages | How It Is Commonly Used |
|---|---|---|
| Lower-strength daily product | 5 mg | Often used as a regular once-daily treatment for ED, BPH or both when prescribed that way. |
| Higher-strength product | 10 mg or 20 mg | More commonly associated with as-needed treatment for erectile dysfunction. |
The two approaches should not be combined casually. Someone who already takes tadalafil every day should not add a higher-strength tablet simply because a particular occasion seems to require a stronger effect.
Why Tadalafil Lasts Longer Than Many ED Medicines
Tadalafil has a relatively long duration of pharmacologic effect, which is why it is sometimes called the “weekend” ED medicine. Its activity can remain clinically relevant for up to about 36 hours in some patients, considerably longer than sildenafil and several other PDE5 inhibitors. This does not mean that an erection continues for that entire period, and it does not create a constant sexual response. Tadalafil still depends on sexual stimulation, while the longer window simply gives the medicine more time in which it can support the normal erectile response.
The same long duration affects safety as well as convenience. Side effects such as headache, flushing, indigestion, back pain or muscle aches can remain relevant for longer after a dose, and interactions with nitrates, alpha blockers or other blood-pressure medicines also need to be considered over a longer interval. Taking sildenafil, vardenafil or another ED medicine during that period can increase the risk of low blood pressure and other adverse effects without producing a predictable additional benefit. Patients who develop prolonged chest symptoms, severe dizziness, sudden vision or hearing changes, or an erection lasting several hours should seek medical attention instead of taking another dose.
Safety Information
Nitrates. Tadalafil must not be used together with organic nitrates such as nitroglycerin, isosorbide mononitrate or isosorbide dinitrate because the combination can cause a sudden and potentially dangerous fall in blood pressure. This applies to nitrates taken regularly as well as those used only when chest pain occurs. Tadalafil remains active for considerably longer than several other ED medicines, so the interaction can still be relevant well after the tablet was taken. Anyone who develops chest pain after tadalafil should tell the treating medical team when the last dose was used rather than taking nitroglycerin independently.
Riociguat and other guanylate cyclase stimulators. Tadalafil is also contraindicated with guanylate cyclase stimulators such as riociguat. Both medicines can lower blood pressure through related signaling pathways, and using them together can produce excessive hypotension.
Blood-pressure medicines and alpha blockers. Tadalafil has a vasodilating effect of its own, so dizziness, light-headedness or low blood pressure can become more likely when it is combined with alpha blockers or other antihypertensive medicines. This deserves particular attention in men already taking drugs such as tamsulosin for urinary symptoms. When tadalafil is being used for erectile dysfunction in someone established on an alpha blocker, the prescribing information recommends a cautious approach to dosing; combining tadalafil with alpha blockers specifically for BPH is treated differently and requires medical review.
Heart and circulation problems. Sexual activity itself places additional demand on the cardiovascular system, so tadalafil should be assessed in the context of the patient’s cardiac health rather than viewed separately from it. Men with unstable cardiovascular disease, recent serious cardiac events or symptoms that make sexual activity medically inadvisable need evaluation before treatment. Chest pain, severe shortness of breath, fainting or another major cardiovascular symptom during sexual activity requires urgent medical attention.
Alcohol. Larger amounts of alcohol can add to tadalafil’s blood-pressure-lowering effect and may increase dizziness, headache, rapid heartbeat or orthostatic hypotension. The risk is more relevant when several drinks are consumed over a short period or when other blood-pressure medicines are already part of the treatment plan.
Common side effects. Headache, indigestion, flushing, nasal congestion, back pain, muscle aches and pain in the limbs are among the reactions reported most often with tadalafil. Back pain and muscle discomfort can sometimes appear hours after the dose because tadalafil remains in the body for a relatively long period. Persistent or unusually severe symptoms should be reviewed rather than repeatedly treated by adding other medicines without checking for interactions.
Prolonged erection and priapism. An erection lasting longer than four hours requires emergency medical attention, whether it is painful or not. Untreated priapism can permanently damage erectile tissue. Additional caution is appropriate in men with conditions that increase this risk, including sickle cell disease, leukemia, multiple myeloma or certain anatomical abnormalities of the penis.
Vision changes. Sudden loss of vision in one or both eyes requires immediate medical evaluation and tadalafil should be stopped while the problem is assessed. Rare cases of non-arteritic anterior ischemic optic neuropathy, or NAION, have been reported with PDE5 inhibitors. A previous episode of NAION should be discussed with the prescriber before tadalafil is used again.
Hearing changes. Sudden hearing reduction or hearing loss has also been reported with PDE5 inhibitors, sometimes together with ringing in the ears or dizziness. A sudden change in hearing after tadalafil should be treated as a prompt medical issue rather than waiting to see whether it resolves after the medicine wears off.
Drug interactions through CYP3A4. Tadalafil is metabolized mainly through CYP3A4, so strong inhibitors such as ketoconazole and ritonavir can substantially increase tadalafil exposure. This can require a lower dose or a longer interval between doses. Strong CYP3A4 inducers such as rifampin can have the opposite effect and reduce tadalafil exposure.
Kidney and liver impairment. Reduced kidney function can slow tadalafil clearance and change how much of the medicine remains in the body, which can require a different dose or dosing interval. Liver disease also affects how tadalafil should be used, and severe hepatic impairment is a situation where current prescribing information does not recommend treatment.
Other erectile-dysfunction medicines. Tadalafil should not simply be combined with sildenafil, vardenafil or another PDE5 inhibitor when the first dose seems insufficient. The safety and benefit of combining ED treatments have not been established, while the effects on blood pressure and other adverse reactions can accumulate.
Before treating urinary symptoms. Men using tadalafil for BPH should have the urinary symptoms evaluated before treatment begins because prostate enlargement is only one possible cause. Infection, urinary retention, bladder disease and other urological conditions can produce a similar pattern and may require a different approach.
Allergic reactions. A previous serious hypersensitivity reaction to tadalafil or one of the product ingredients is a contraindication. Facial swelling, widespread rash, blistering skin, breathing difficulty or another severe allergic reaction requires urgent medical care.
Types of Tadalafil Sold in Mexico
The Mexican tadalafil market extends well beyond Cialis and one standard generic product. Large pharmacy chains carry their own private-label tadalafil beside independent commercial brands, while the original Cialis remains available in several catalogs. A customer searching the same 5 mg strength can therefore encounter products from different laboratories, different package quantities and a substantial spread in price before changing the active ingredient at all.
Cialis remains the best-known original brand, while Mexican pharmacy catalogs also include products such as Invictus and pharmacy-label tadalafil from major chains. This gives the market a structure familiar with many medicines sold in Mexico: an original international brand at one end, branded generics from established laboratories in the middle, and lower-cost pharmacy lines alongside them.
Packaging varies considerably. The 5 mg strength can appear in 14- and 28-unit presentations, while higher-strength products are often sold in much smaller boxes. Looking only at the price displayed beside the medicine name can therefore give a poor comparison, because two tadalafil listings may contain very different amounts of medication.
There are also less familiar pharmaceutical forms. Adial Odifs 5 mg from Laboratorios Silanes, for example, is sold in individual sachets rather than as an ordinary box of conventional tablets. Products like this make the Mexican catalog noticeably broader than a simple choice between “Cialis or generic tadalafil,” and the exact dosage form, manufacturer and unit count should be checked before two listings are treated as equivalent.
Tadalafil + Tamsulosin
Mexican pharmacy catalogs also include fixed-dose products that combine Tadalafil 5 mg with Tamsulosin 0.4 mg in the same treatment. This type of product is aimed at men whose lower urinary tract symptoms associated with BPH may benefit from both medicines, particularly when erectile dysfunction is also part of the clinical picture.
The two ingredients work through different mechanisms. Tadalafil is a PDE5 inhibitor and can improve erectile function while also helping some urinary symptoms associated with BPH. Tamsulosin is an alpha-1 blocker that relaxes smooth muscle around the prostate and bladder outlet, making urination easier. Putting both into one product creates a different treatment from ordinary tadalafil 5 mg or standard tamsulosin 0.4 mg used alone.
The combination also brings the precautions of both drugs into the same prescription. Tadalafil and tamsulosin can each lower blood pressure, so dizziness, light-headedness or symptomatic hypotension can become more relevant when they are used together, particularly during treatment initiation or when other antihypertensive medicines are already being taken. A patient using a fixed combination should also check the rest of the medication list carefully to avoid accidentally adding separate tadalafil or tamsulosin on top of the same ingredients.
For customers searching Mexican pharmacies, the product name alone can be misleading because “tadalafil 5 mg” may appear prominently even when tamsulosin 0.4 mg is included in the same formulation. The complete ingredient list should therefore be checked before comparing the product with Cialis, generic tadalafil or a separate tamsulosin capsule.
